Short version: the commonly quoted PKR 50–80 lakh "all-in" figure is real as far as it goes — it covers tuition, dorm, and living at budget-to-mid-tier universities. What it leaves out is the NRE tail: Pakistan's licensing exam for foreign graduates had a 15.5% per-sitting pass rate for China-based candidates in December 2025, most graduates need more than one sitting, and every sitting adds exam fees, prep months back home, and a house-job stipend you are not earning. Budget a range with an NRE reserve, not a single number.

1. What the standard quote actually buys

The fee structure breakdown on this site works through the components. Compressed, the honest six-year bill has two layers:

Add both layers and the agent consensus of PKR 50–80 lakh for a complete six-year budget-to-mid-tier degree is internally consistent. A premium Beijing track is higher: CCMU-billed plus Beijing living gives roughly ¥94,000–122,000/year ≈ PKR 3.9–5.0 million, i.e. about PKR 23–30 million over six years. None of this is the controversial part.

2. The part quotes leave out: the NRE tail

After graduation, a Pakistani with a Chinese MBBS cannot register or practice at home without passing Pakistan's National Registration Examination (NRE) — Step-I and Step-II, run under PMDC/NUMS. Per-sitting, most candidates fail. That single statistic creates a list of costs that no agency package covers:

One honest caveat in both directions: there is no cap on NRE attempts (Section 18(1) of the PM&DC Act 2022, per third-party summaries), so the tail is a delay and a cost — not, for most families, a dead end. And the 15.5% figure cuts both ways: some graduates pass on the first sitting. The honest statement is the distribution, not a horror story.

3. Three planning scenarios

These are planning aids, not predictions. Assumptions: one NRE sitting ≈ PKR 20,000 exam fee; prep months spent at home at Pakistani living costs; "foregone stipend" left qualitative because house-job pay varies.

The point of the table is not the exact numbers. It is that "MBBS total cost" is a range whose width you partly control, and that the width comes from NRE preparation, not from tuition.

4. Compared with staying in Pakistan — stated fairly

A Pakistan private medical college runs about PKR 1.2–2 crore over six years; PMDC capped private-college fees at PKR 1.8 million/year for 2025–26 (~1.89 million with the permitted increment). So the sticker comparison is: China budget-to-mid-tier (PKR 50–80 lakh) against Pakistan private (PKR 1.2–2 crore plus living). China usually wins on upfront cost.

But fairness requires two footnotes. First, the NRE tail is specific to foreign degrees — a local graduate does not sit it, so do not compare China's quote-plus-tail against Pakistan's quote alone; compare worst cases against worst cases. Second, the gap narrows at the premium end: a PKR 23–30 million Beijing track and an expensive Lahore/Karachi private seat are closer than the marketing suggests. For a budget-to-mid-tier university: plan around PKR 50–80 lakh for the degree itself, plus an NRE reserve of PKR 0.5–1.5 million and 12–18 months of post-graduation runway. A premium Beijing track pushes the degree portion toward PKR 23–30 million.

Is the agent lying when they quote PKR 50–80 lakh?

Not necessarily lying — just stopping early. That figure reasonably covers tuition, dorm, and living. It simply does not include the licensing exam that stands between the degree and a Pakistani practice licence.

Does the NRE apply if my university is PMDC-recognized?

Yes. PMDC recognition of your university is a precondition for eligibility, not an exemption. Every graduate of a foreign medical school — recognized or not — must pass the NRE to register in Pakistan.

What happens to graduates who never pass?

There is no attempt cap, so most keep sitting. Some eventually pivot — research, hospital administration, public health, or work abroad where a Chinese degree plus local requirements open other doors. That pivot is the real cost of an unplanned tail, and it is why we tell families to budget the reserve before they celebrate the admission.
